Naples Urology Associates closure effective October 25, 2024. Dr. Joanna Chon will be transferring to Advanced Urology Institute (AUI) located at 1035 Piper Blvd. Suite 101, Naples, FL 34110, (239) 465-4157. This is to notify you that GenesisCare USA will be transferring medical records of Dr. Joanna Chon to Advanced Urology Institute.

If you do not want your medical records included in the transfer, you may contact Datavant by emailing 78560_Genesis_Care@datavant.com or faxing a release to (256) 429-2795 to request a copy of your records or to request that they be transferred to another provider.
