This shall constitute notice that the dental practice known as Coast Dental Vero Beach owned by Coast Dental of Florida P.C., and located at 528 Miracle Mile Blvd

Vero Beach, FL 32960, is closing effective February 1st, 2025. All patient dental records located at Coast Dental Vero Beach will be transferred to Coast Dental Corporate located at 5706 Benjamin Center Drive, Suite 103, Tampa, FL 33634. The Coast Dental Corporate phone number is (877) 880-1212. Patients can request their dental records by sending a written request to the Coast Dental Corporate address. The patient may be billed for actual costs incurred for copying, mailing, or delivering the records as permitted by law.
