OF MEDICAL PRACTICE

NOTICE is hereby given that Allergy Asthma Immunology Care of Central Florida terminated its practice on 5/29/2026.

Patients may obtain a copy of their medical records by submitting a records release form located on our website at:

www.andrademd.comand fax it to: (407) 658-7995.

Records will be available for transfer or pick up until June 30, 2026. After this date, records will be maintained by a records custodian company, which will be announced on our website.

A copy of this notice has been submitted to the Florida Board of Medicine.
