NOTICE: RELOCATION OF PRACTICE

Notice is hereby given that, Jamaal McLeod, D.O., formerly practicing at Fountain Life, 6424 Alexandra Louise Dr Ste 120, Orlando, FL 32827, has left that practice effective of June 30, 2026. Dr. McLeod will continue practicing at a new, independently owned practice; Zenith Wellness, LLC. Patients' medical records remaining at Fountain Life will be retained for a period of not less than two (2) years and may be obtained by contacting Fountain Life at 407-499-8511 or sending in a written request to, Fountain Life, 6424 Alexandra Louise Dr Ste 120, Orlando, FL 32827.
