2027 COMMUNITY CARE FOR THE ELDERLY (CCE), HOME CARE FOR THE ELDERLY (HCE), ALZHEIMER'S DISEASE INITIATIVE (ADI) REQUEST FOR PROPOSALS FOR COMMUNITY CARE SYSTEM LEAD AGENCY DESIGNATION IN INDIAN RIVER, MARTIN, OKEECHOBEE, AND ST. LUCIE COUNTIES AND

PALM BEACH COUNTY SERVICE AREAS A AND B

The Area Agency on Aging of Palm Beach/Treasure Coast, Inc. is requesting proposals from organizations that have the operational capacity and interest in being designated as the Lead Agency for the Community Care System for each of the Community Care Service Areas (CCSAs) in Planning and Service Area 9 beginning July 1, 2027. The CCSAs include Indian River, Martin, Okeechobee, and St. Lucie counties as well as Palm Beach County Service Area A and Palm Beach County Service Area B. Separate proposals are being requested for each CCSA for the provision of services to all eligible CCE, HCE, and ADI clients in the CCSA. Organizations interested in obtaining Community Care System Lead Agency designation are requested to submit written proposals. An original hard copy plus one additional hard copy, and an electronic copy of their proposal on USB drives must be submitted in a securely sealed envelope or container addressed to Area Agency on Aging of Palm Beach/Treasure Coast, Inc., 701 Northpoint Parkway, Suite 115, West Palm Beach, FL 33407. All proposals must be submitted prior to 12:00 PM November 17, 2026. Proposal format and instructions are available on the website www.aaapbtc.org.
